License Verification. To request certified license verification be sent to another entity ($25 fee) 

If you need certified license verification sent to another entity, for a $25.00 fee, you may request licensure certification ONLINE or IN WRITING.

To Request Licensure Public Records – (Requests do not have to be in writing)
If you are unable to obtain the information or documentation you need from the applications above, you may submit a licensure public records request by contacting the Department at (850) 245-4121, completing the online form or submitting a request in writing (fees may apply). Public records requests are not required to be in writing. You do not have to give your name or explain why you are making a request.

Scores
If you are a licensee and cannot find your scores via the Exam Scores Search page and would like to have your scores mailed to another State, please fill out the score waiver and submit with your public records request in writing (fees may apply).
