When did the Florida Arthritis Prevention and Education Program begin and what services does it provide?

The Florida Arthritis Prevention and Education Program began through a four-year cooperative agreement with the Centers for Disease Control and Prevention (CDC) in September 1999. The current cooperative agreement with the CDC started in July 2008 and will end in June 2012. Under this cooperative agreement, the Arthritis Prevention and Education Program:

  • Promotes the expansion of evidence-based self-management and physical activity programs recommended by the CDC into delivery systems. These programs are recommended for people with arthritis and other chronic health conditions.
  • Develops partnerships to expand and sustain the CDC-recommended self-management and physical activity programs regionally and statewide.
  • Helps community partners develop delivery systems for the CDC-recommended self-management and physical activity programs by providing support for trainings, licenses, and start-up equipment needs.
  • Provides technical assistance to state and community organizations that offer the CDC-recommended self- management and physical activity programs.
  • Maintains a statewide data collection system for the CDC-recommended self-management and physical activity programs.

What is an evidence-based program?

Evidence-based programs refer to programs that have been shown to improve the quality of life of people with chronic health conditions. The evidence supporting these programs is based on scientific research that has been peer reviewed. Evidence-based programs have:

  • A specific population.
  • Specific, measureable goals.
  • A stated reasoning and proven benefits.
  • A well-defined program structure and timeframe.
  • Specific facility and equipment needs.
  • Specific staffing needs and skills (can include community volunteers).
  • A built in program evaluation to measure program quality and health outcomes.

The self-management and physical activity programs promoted by the Arthritis Prevention and Education Program are evidence-based for individuals age 18 years of age or older.

What is a delivery system?

A network of community organizations working together to expand and sustain evidence-based self-management and physical activity programs.

What can evidence-based programs do for me?

Research shows that people with arthritis and other chronic health conditions who participate in the CDC-recommended, evidence-based self-management and physical activity programs can result in:

Physical Activity Programs

The Arthritis Prevention and Education Program promotes five physical activity programs, which include:

Where can I find evidence-based programs in my community?

The Arthritis Prevention and Education Program and its partners have developed delivery systems for the CDC-recommended self-management and physical activity programs. These 10 delivery systems cover all 67 counties in Florida. Each delivery system includes several counties in an area and are self-sufficient in providing both human and financial resources to maintain and sustain the programs.

To find which CDC-recommended self-management and physical activity programs are available in your county and to find out who to contact, click on a county in the map below. An area may have up to three delivery system partners. These will be listed as Delivery System Partner A, Delivery System Partner B, and Delivery System Partner C in the map's popup window. For areas that only have one delivery system partner, there will only be information listed for Delivery System Partner A. You may have to scroll up in the popup window to see this information.
