Asthma Action Plans

An asthma action plan is a written, individualized worksheet that shows the patient and caretakers the steps to take to prevent your asthma from getting worse. The National Asthma Education and Prevention Program's Guidelines for the Diagnosis and Management of Asthma (EPR-3 Guidelines) recommend that health care providers caring for patients with asthma work with them to develop an asthma action plan.

• Asthma Control: Home-Based Multi-Trigger, Multicomponent Environmental Interventions
  The Task Force on Community Preventive Services recommends recommends the use of home-based multi-trigger, multicomponent interventions with an environmental focus for children and adolescents with asthma based on evidence of effectiveness in improving overall quality of life and productivity, specifically:
  • Improving asthma symptoms
  • Reducing the number of school days missed to asthma
• Asthma Home Environment Checklist
  This checklist guides home care visitors in identifying environmental asthma triggers most commonly found in homes.  It includes sections on the building, home interior and room interior and provides low-cost action steps for remediation.
• Clearing the Air: 10 Steps to Making your Home Asthma-Friendly (Spanish Version)
  This one page, simple to follow guidance document lists recommended actions to help control asthma triggers in the home.
